Team Health CheckSprint retrospective template
Six-dimension team check-in — spot drift before it becomes a problem.
Columns in this template
A structured check-in on six dimensions: delivery pace, quality, teamwork, learning, energy, and fun — to spot drift before it becomes a problem.
When to use Team Health Check
Team Health Check is built for teams that want more than process feedback — they want a read on how the team itself is doing. Thriving/Steady/Struggling maps the overall health across delivery, quality, collaboration, energy, learning, and morale. The Ideas column immediately turns health signals into concrete experiments, preventing the retro from ending on a diagnosis with no treatment plan.
Facilitator tips
- Ask the team to think across six dimensions: delivery pace, quality, teamwork, learning, energy, and fun. Prompt each explicitly if the board feels thin.
- Steady is not mediocre — help the team see it as a stability signal worth protecting.
- Compare Struggling themes to last session's Team Health Check if you run this format regularly. Pattern recognition across sprints is where the real insight is.
